I, the undersigned, do hereby voluntarily submit my application for participating in the HWA-RANG FOUNDATION INVITATIONAL TOURNAMENT to be held October 16, 2010 at Sunnyside United Methodist Church, 3520 S.E. Yamhill St., Portland,OR 97214. I assume full responsibility for any and all damages, injuries or losses which I may sustain or incur while attending or participating. I hereby waive all claims against the promoters, the sponsors, the building owners, Sunnyside United Methodist Church or any individuals for injuries I may sustain. I fully understand that my entry fee is NON-REFUNDABLE. I HAVE READ AND FULLY UNDERSTAND THE ABOVE WAIVER.
